Two Ways to Help Your Patient Enroll
Completing the enrollment form takes about 15 minutes, online or offline. Before you start please gather all required patient information and medical and pharmacy insurance cards.
Enroll Online: You and your patient sign the forms electronically. Please also upload your patient's medical and pharmacy insurance cards. Your patient will receive an email requesting their signature to complete the process.
Print Enrollment Form: Completed form must be faxed with copies of the patient’s medical and pharmacy insurance cards to 1-800-645-9043.
If applying online there is no need to set up an account or password. Please note that the information you enter in the form is not saved when you close it.
Copay Assistance
Eligible commercially insured patients could pay as little as $0 up to a total of $26,000 for up to 12 months for their medicine. Download and complete the Together with GSK Oncology enrollment form with your patient to submit for approval. Terms and Conditions apply.
Patient Assistance Program (PAP)
Uninsured patients who meet eligibility requirements may access medication free of charge through the GSK Patient Assistance Program (PAP) offered by the GSK PAP Foundation. Medicare patients who meet program requirements may also be eligible for the Patient Assistance Program. Check Eligibility.
